Injury and Illness Reporting
WHAT TO REPORT Reporting through BGSU's electronic form is required for:
- All injuries which are work or educational related regardless of where they occur (on or off campus); AND
- Any injury on BGSU property.
- All illnesses which are work or educational related regardless of where they occur (on or off campus); for example, reporting personal medical incidents that would not have been caused by the individual’s work or educational activities or environment need not be reported.
WHO SHOULD REPORT We encourage the injured or ill party to complete the online form but realize that is not always possible. Therefore, anyone from BGSU with a sign-on user name and password who has intimate knowledge of the incident can submit a report on behalf of that person.
DO NOT submit a completed version of this form to our department. The intent behind this voluntary form is to provide the ability to collect information in the field for incidents involving a visitor, contractor, student, etc. It is NOT intended for use with injured/ill employees who were working at the time of the incident (if the employee is NOT acting under “employee” status at the time, i.e. “visitor”, this form can be used). Following the usage of this form, the information shall be submitted electronically using the University’s online application found above by clicking the large gray box stating, “Submit a Form”.
