Injury and Illness Reporting

WHAT TO REPORT  Reporting through BGSU’s electronic injury/illness form is required for:

  •   All injuries/illnesses which are work or educational related regardless of where the injury/illness occurs (on and off campus); and
  •   All injuries/illnesses on BGSU property.   

Exemptions:

  1. Student athletes injured while participating in NCAA athletic activities.
  2. Personal medical incidents that were not caused by the individual’s work or educational activities (if unsure, report).

WHEN TO REPORT Within twenty-four hours of the incident, or as soon as possible following the incident or knowledge of the incident. If there is a fatality, inpatient hospitalization, amputation, or loss of an eye, call EHS immediately at 419-372-2171 and leave a message if it is after hours or if you reach voicemail during normal business hours. Public employers are required to report these types of incidents to PERRP (Public Employment Risk Reduction Program) within 8-24 hours depending on the type of incident.

WHO SHOULD REPORT  It is encouraged that the injured or ill party complete the online form. If this is impossible or will result in an extended delay in reporting, anyone with intimate knowledge of the incident, who has BGSU login credentials, can submit a report on behalf of the injured/ill party. Reports for guests are to be reported electronically by the hosting department or any other BGSU-affiliated person.

    DO NOT submit a completed version of this form to Environmental Health and Safety. The intent behind this temporary form is to provide staff the ability to collect information in the field for incidents involving visitors, contractors, and students. It is NOT intended for use with BGSU injured/ill employees who were working at the time of incident. Following the usage of this form, the information must be submitted electronically using the university’s online form (“Submit a Form” box above).
