#VaxBGSU Student Vaccination Incentive Program Official Rules

Eligibility

You are eligible to participate in weekly drawings if, at the time of the drawing:

  1. You are a student at the University enrolled for spring or fall semester 2021
  2. You are at least 13 years of age.
  3. You are not a member of the University Board of Trustees, child/step-child, immediate blood relative, or member of the household of a member of the University Board of Trustees or President’s Cabinet.
  4. You have received the first dose of a COVID-19 vaccine that has been authorized for use in the United States by the U.S. Food and Drug Administration or internationally by the World Health Organization. (Note: In order to claim a prize, you must have received and submitted proof of the final recommended dose of a COVID-19 vaccine (e.g. the second dose of a two-dose vaccine or the first dose of a one-shot vaccine))
  5. You have submitted proof of your vaccination information with the University through the Med+Proctor portal link located on MyBGSU.

The winners of the drawings will be randomly selected from among all eligible entries received.

Subject to verification of eligibility and compliance with these Official Rules, the potential winners drawn and verified will be declared the official winners of the Promotion drawings.

Entry

Eligible students may enter BGSU’s vaccination prize drawing ("Promotion") through September 27, 2021. Student who have already submitted proof of vaccination to the University through the Med+Proctor portal are already entered into the drawing.

The drawings will be conducted weekly, for eight (8) weeks, beginning on August 11, 2021.

There will be one (1) drawing each week. Information about each week’s winners will be posted on the #VaxBGSU website in accordance with the announcement schedule above.

Students may enter as soon as they have received the first dose to meet the eligibility requirements. Prizes cannot be claimed until student has completed the second dose of a two-dose vaccine. All entries will be included in all drawings for the Promotion after the entry is received, unless you win, in which case you will be removed from future drawings.

By successfully submitting proof of vaccine through the Med+Proctor portal link on MyBGSU each participant will be entered into the Promotion. Students who do not have access to the internet may contact 419-372-3000 for assistance entering drawing. You must provide the requisite information and request that you be entered into the drawing. Entries must be received by the Entry Due Dates listed above for each drawing.

Neither the University nor its Board of Trustees or any of its employees, boards, officers, affiliates, contractors, assignees, agents and representatives are responsible for late or incomplete or misdirected entries, or responsible for incorrect or inaccurate information, whether caused by the website, or by any equipment or programming associated with or utilized in this Promotion.

By entering the drawing, you explicitly authorize the University to access your vaccination record, which was voluntarily entered by you to the Med+Proctor portal link located on MyBGSU, to verify that you have been vaccinated against COVID-19. Proof and verification of vaccination, to the satisfaction of the University, is required.

One entry per person. Eligible students can submit only one (1) entry into the drawings based on their primary affiliation – either as a student or as an employee. Prior to the drawings, all duplicate entries will be removed. All winners will be removed from future drawings. No participant may win more than one prize.

Verification

Verification of all information, including proof of having received the final recommended dose of a COVID-19 vaccine, must be presented to the University to claim any prize. Any information registered for or by a minor child or an adult under a guardianship must be validated by a parent or legal guardian prior to award of a prize.

In the event that an entry is deemed invalid, a discrepancy is found, or the winner cannot be located based on the contact information submitted, an alternate winner shall be used in place of the person initially selected. If it is determined that an alternate will be used, they will be contacted in the order in which they were selected. Individuals may be declared ineligible if they have not provided all necessary information by 2 weeks after a drawing. The promptness and degree of reasonableness of efforts to locate a winner or of any decision to deem a winner ineligible based on these rules shall be at the sole discretion of the University.

In the event that a winner does not wish to receive a prize, refuses or fails to provide timely verification information or the required releases, or does not promptly respond to the initial contact by University officials, that winner may be deemed ineligible and an alternate winner may be used. The person initially selected will forfeit any prizes associated with the Promotion.

The university reserves the right to request additional documentation should there be any questions regarding the vaccination record. If you are unable to be vaccinated due to a religious or medical reason and would like to enter the Promotion, please contact 419-372-3000.

Grand Prize

The grand prize is in-state tuition and fees for the remaining consecutive semesters of the current level of degree (undergraduate or master’s) being pursued by the winning student. This prize is valued at in-state actual assessed tuition and fees, not to exceed the full-time (12-18 credit hours) amount of the winner’s Falcon Tuition Guarantee Cohort rate for a maximum of eight semesters, regardless of the student’s level or field of study. If the winner is not in a Falcon Tuition Guarantee Cohort, the prize will cover the assessed in-state tuition and fees until graduation or for a maximum of eight semesters. Students receiving University scholarships and grants that cover full cost of attendance are not eligible for scholarship prizes but may accept experience prizes. Students receiving employee dependent benefits are eligible only for the non-covered portion of their tuition and fees. Federal student aid recipients may experience adjustments in their overall financial aid package. All or part of the awards may be taxable. 

General Rules

All prize details shall be determined in the sole and absolute discretion of the University. Students who do not meet the eligibility requirements defined in these Official Rules are not eligible to win.

Prizes are nontransferable to any other person, including relatives or friends.

Winners will be responsible for any taxes associated with their prize. Federal, state and local taxes are the sole responsibility of the applicable official winners. All winnings will be reported to the official winners and the IRS, as required by law. All federal, state and local laws, applicable taxes, and regulations apply. A student may not receive financial aid and other resources in excess of the annual cost of attendance at the university. Student Financial Aid will work with student winners to maximize the amount they can accept based on their cost of attendance and other factors.

The drawing prize may be reduced by a past due balance owed to the University and/or the Attorney General’s Office, if applicable.

BGSU’s vaccination incentive program’s Official Rules may be amended or revised at any time at the sole discretion of the University. Winners will be selected through random drawings that will be overseen by the Office of the President.

The University, at its discretion, reserves the right to select and/or change the prize(s) to be awarded as part of this Promotion or end the Promotion prior to all prize winners being drawn.

All information submitted online by entrants is subject to, and will be treated in a manner consistent with the University’s Sensitive Data Privacy Policy.
