ACCIDENT REPORTING AND INVESTIGATION REPORTS

The Injury and Illness Report must be completed for all injuries and illnesses to employees and non-employees which occur on BGSU property. This report also applies to injuries and illnesses that happen to BGSU employees who are on work assignments off BGSU property. The second side of this two-sided white form must be completed only for BGSU employees, and only for the situations given in the instructions.

The Bowling Green State University Accident Investigation Report is encouraged but optional. This report is a tool for managers, supervisors, and accident investigation teams to find the causes of injuries, illness, and "near misses" and to take corrective actions.
