Applying for Services
To apply for accessibility services, students must submit documentation of their disability for review by AS staff. There is no deadline to apply for services, but students should be advised that the review process can take several weeks. We encourage prospective students to submit their documentation as soon as they have been accepted to BGSU and have a BGSU ID number and BGSU email address.
Steps to Applying for Services
- Complete a student application for services - Apply for Accessibility Services by filling out the relevant application for services - academic, dining, or housing. Each application is separate. Please only complete an application if you are requesting an accommodation in that area on the basis of a documented disability.
- Once an application has been submitted, you will receive an email with a link to our paperwork. Please have your medical provider complete our disability verification form or submit equivalent information. Be sure to include copies of your most recent high school IEP, 504 plan, and multi-factored evaluation (MFE), if applicable.
