Camp Program Information

Parental, Medical Consent and Waiver Forms

In order to assist departments in planning a summer conference or camp program for minors, Risk Management, in conjunction with the Student Health Service, has developed the following Parental, Medical Consent and Waiver forms:

• Medical History and Consent for Emergency Medical Treatment
• Permission to Dispense Medication by Camp Program Staff
• Immunization Record (applicable only for residential camps, i.e., those requiring an overnight stay in a residence hall)
• Liability Release, Waiver, Discharge and Agreement Not to Sue (for minor participation)
