         A valid State-issued driver's license with photo.
A valid State-issued photo ID card
         A valid passport with photo

*Please note: If you have never been issued one of the aforementioned forms of identification, please see this document for more information.  Additionally, if you are a College Credit Plus student age 0-17 years that has never been issued a primary form of identification, please have your parent or legal guardian complete this form and accompany you to the BG1 Card Services office.

Your BG1 Card is used to verify your identity and to complete financial transactions. You should guard your BG1 Card like cash or a credit card in order to keep it safe and secure at all times.

You will be charged a fee to replace a lost/stolen/damaged card, so take good care of your card!

If you'd prefer not to submit your application online, simply visit our office during normal business hours to have your picture taken and your BG1 Card printed.  You will still need to have an active BGSU Account, be enrolled in classes/entered as an employee by Human Resources, and (if applicable) provide a completed Department Authorization Form prior to visiting our office.

Please DO NOT:
lend your card to other people
        mark or bend your card
        punch holes in your card
scratch the magnetic strip on your card
use your card as an ice scraper

You can report a lost or stolen card 24/7 by clicking on BG1 Card Services at MyBGSU

As long as you have an active BGSU Account and are registered for classes (current or future term), you can follow the above link and submit your request.  Once submitted and approved, your card will be ready and waiting for pickup during normal business hours in our office.  The non-refundable $25 fee for all new or replacement cards may be posted to the student's Bursar Account or paid in the office with cash, check, American Express, Discover, Master Card or Visa.

Most new students pick up their first BG1 Card during Student Orientation, Advising and Registration (SOAR).  After you complete your BGSU Account Registration and prior to SOAR, you have the ability to submit the photo you’d like used for your BG1 Card.  Once submitted and approved, your card will be ready and waiting for pickup during SOAR.  Please submit your photo at least one week prior to your scheduled SOAR date so we can ensure your card is approved and printed before your arrival on campus.  If you are not attending SOAR, please follow the instructions above under the “New Students / Existing Students / Replacement Cards” heading.  There is a $25 non-refundable fee to get your BG1 Card. The fee for your initial card will be automatically posted to your Bursar account along with your tuition and other fees.

After you have been entered as an employee by Human Resources, established a BGSU Account, and obtained the completed department authorization form to cover the $25 fee for the initial card, you may follow the link above to submit your request for a new BG1 Card.  Once submitted and approved, your card will be ready and waiting for pickup during normal business hours in our office.  The non-refundable $25 fee for replacement cards is the responsibility of the individual and may be posted to the faculty or staff member’s Bursar Account or paid in the office with cash, check, American Express, Discover, Master Card or Visa.
