Student Laptop Loan Agreement

Information Technology Services (ITS) hereby loans to Borrower the following device: 
 
I hereby agree to the following terms and conditions when accepting loaner equipment from BGSU Information Technology Services (ITS). I understand that the computer system provided and related equipment I am receiving is a loan, and accept any charges incurred for any loss or damage to said equipment. The loaned laptop and additional accessories including the power cable cannot be taken out of the United States. The student borrower should plan on returning the laptop if traveling out of the country. I also understand that to remain eligible, I must be enrolled in at least one BGSU course and the laptop loan device plus AC adapter (and hardshell case, if provided) must be returned at the end of the Academic Year. 
 
1. PHYSICAL DAMAGES:

Student Borrower is fiscally liable for any physical damage (including liquid damage) incurred to the laptop, AC adapter, and/or hard-shell case (MacBooks only) provided during the loan Period. If physical damage has been incurred to the laptop, AC adapter, and/or hard-shell case, ITS will work with the Student Borrower to determine the cost of repair or replacement. Charges for repairs or equipment replacement will be charged to Student Borrower’s BGSU Bursar account. Student Borrower also agrees to refrain from applying decals/stickers or doing other modifications to the laptop, as well as refrain from removing BGSU or manufacturer-applied decals/stickers. 
 
2. LOSS OR THEFT:

ITS does not assume responsibility for theft or loss of the laptop. A stolen or missing laptop should be reported immediately to BGSU Campus Police at 419-372-2346. As soon as a police report is available, you must provide a copy to ITS. Student Borrower is fiscally liable for the full value of the laptop. If the laptop is not recovered, a charge for the full value amount will be applied to Student Borrower’s Bursar account. If provided AC adapter is lost, this should be reported to BGSU ITS. Student Borrower will be charged the replacement cost, based on the type of laptop loaned. Student Borrower may opt to make this payment directly to BGSU ITS using BG1 Bucks. 
 
Dell: AC Adapter- $45 
 
MacBook: AC Adapter - $59, USB charging cable - $49 
 
MacBook hard-shell case: $20 
 
3. TECHNICAL SUPPORT:

BGSU ITS will field and support issues related to the following, but not limited to: pre-installed software, operating system, login issues, network connectivity, and/or warranty-covered hardware failure. This support will be provided to the Student Borrower, free of charge, but will be subject to standard support turnaround times. 
 
4. EARLY DISMISSAL OR WITHDRAWAL FROM BGSU:

If Student Borrower is dismissed or withdraws from classes at BGSU prior to the end of the Loan Period, Student agrees to return Laptop to BGSU ITS immediately. 
 
5. LATE FEES:

The Student Borrower will be charged $50.00 per day past the due date to Student Borrower’s BGSU Bursar account. Late fees will be applied daily until the laptop is returned or the full value of the loaned device is reached. 
 
FAILURE TO RETURN LAPTOP:

After five (5) attempted contacts made with Borrower past the due date, ITS will contact BGSU Police for assistance to recover the laptop. If Borrower is no longer affiliated with BGSU or cannot be located to recover the laptop, criminal charges may be pursued. 
 
**Please note any visible physical damage currently present on the laptop below, including provided AC adapter and reply immediately.** 
 
I have read, fully understand, and consent to the terms of this Laptop Loan agreement.
