Vendor Equipment Loan Waiver

Contracted vendors wishing to borrow specialized equipment, such as lifts and other heavy equipment to perform construction, installation, repair or maintenance work on the BGSU campus must have a completed Vendor Equipment Loan Waiver on file with the Risk Management Department prior to utilizing equipment.

The waiver must be signed by an authorized agent of the vendor, and submitted to Risk Management by the contracting BGSU Department. Signed waivers are good for one year.
