Department Authorization Form for BG1 Card/ID Badge
An authorization form, signed by the budget administrator, is required to charge a department for an individual’s BG1 Card or ID badge. A separate form must be filled out for each individual. The completed form, along with a form of identification, will be required when obtaining a BG1 Card or ID badge at BG1 Card Services. Visit the website for a list of acceptable forms of identification. New employees must be entered as an employee by Human Resources as well as have an active BGSU email account before a BG1 Card can be printed.
Complete the deposit form, enclose payment, and return/mail to:

A Parent/Guardian Statement of Identification form can be used to identify College Credit Plus students age 0-17 years that have never been issued a primary form of identification. This form cannot be used for Undergraduate Students or if a minor has been issued a primary form of identification, but does not have it in their possession.
A student and their parent/guardian can request this form at the BG1 Plus office located in the student union.
Distance Learning Students - BG1 Card Application
Students who are officially identified as Distance Learning Students can apply to receive a BG1 Card. The cards will be printed without a photo. Students must be registered for classes for the current or future terms. The $25 charge will be applied to the Bursar account.

BG1 Card Services would like to encourage all faculty, staff and students to read through our cardholder agreement and disclosure statement regarding the BG1 Card.
BG Bucks Refund Request
Cardholders who withdraws from the University, graduates, or no longer works at the University can request a refund of their BG Bucks. Students who are continuing with the university can request refunds at the end of the spring semester. The refund process takes approximately 3-4 weeks and is sent in the form of a check to the cardholder’s address. A $10 processing fee will be assessed. If the balance on the BG Bucks account is below $10, a refund will not be processed.
Payroll deduction form
One of the many benefits a faculty or permanent staff member of BGSU receives is the option to use payroll deduction to make deposits to their BG Bucks account! The individual determines the amount that Payroll is authorized to deduct from each paycheck. Participation is completely voluntary and can be changed or stopped at any time!
Department contacts who need to access reports from the Transact software need to reach out to bg1plus@bgsu.edu to start that process.
