Faculty and staff in the College of Health and Human Services are dedicated to helping students achieve their education goals as they pursue careers in health and human services. Here are links to resources that can help you during your time as a student in the College. All forms (or requests made via the forms) are subject to college approval.
Have you discussed your decision with your advisor? Please follow the instructions below when completing a form:
1. Download PDF OR choose link
2. Complete fillable portions of form with appropriate information
3. Submit link; OR
4. For PDF:
a. For Windows computers:
i. File > Print > Save as PDF (in printer drop-down menu)
b. For Mac computers:
i. File > “Export to PDF”
*Using the “Save” function will erase any filled information in the document, returning it to the blank and once again fillable document*
