Program Review Fillable Self-Study Master Template

This page provides the program review self-study template which is made available to programs in a fillable Microsoft Word format. Academic programs are required to use this template when preparing their self-study as part of the program review process.

The template is designed to support a comprehensive and consistent evaluation of program quality, effectiveness, and outcomes. As indicated within the document, certain data elements will be provided by the Office of Institutional Effectiveness (OIE) and the program. Programs are responsible for reviewing the data provided and supplying the analytical narrative responses, interpretation of findings, and the development of an action plan that addresses areas for improvement and future priorities.

Detailed instructions for completing the template, including guidance on responsibilities for data and narrative responses, are provided on the cover page of the document. Programs should review these instructions carefully before beginning the self-study.
