Self Service Electronic 1095-C

Electronic Form 1095-C is available at Tax Form Management

Bowling Green State University is required by the Affordable Care Act (ACA) to furnish all eligible employees with a 1095-C for each calendar year to be used in completing the employees' annual tax returns.  

 

Benefits of Receiving Form 1095-C Electronically

There are multiple benefits to receiving an electronic Form 1095-C:

  • Earlier access to the Form 1095-C.
  • No possibility an employee's Form 1095-C might be lost, stolen, delayed or misplaced by the U.S. mail service or by the employee, once received.
  • If an employee misplaces their 1095-C, they can obtain another copy the same day.
  • Access can be attained even if the employee is not presently at his/her residence, for example, if participating in a University foreign program.  Access can be attained from any PC with internet connection.

Disclosure Notices

An employee who consents to receiving his/her Form 1095-C electronically will not receive a paper copy of the 1095-C. If an employee does not consent, he/she will continue to receive a paper copy of the 1095-C. Once an employee has signed up, the consent is valid for all subsequent years unless the employee chooses to withdraw consent.

An employee who chooses to receive his/her Form 1095-C electronically can also receive a paper copy of the 1095-C by contacting the Human Resources in writing. This may be done via email to: benefits@bgsu.edu or U.S. Mail to: Office of Human Resources, Bowling Green State University, 1851 N. Research Dr., Bowling Green, OH, 43403. Request for a paper copy does not withdraw the employee's consent for electronic delivery of future 1095-C statements.

An employee may withdraw his/her consent by at anytime through the online system. If consent is withdrawn, it will only be effective for any Form 1095-C's not yet issued.

The University will not provide the Form 1095-C electronically to an individual who has terminated his/her employment with Bowling Green State University. These individuals will receive paper copies via U.S. mail.

Employees may continue to make any personal information changes (i.e. address changes) as needed through the MyBGSU self service web portal.

The hardware and software requirements needed to access, print and retain Form 1095-C electronically include an internet connection, web browser, and Adobe Acrobat reader.

The Form 1095-C is required to file your income tax return.
