Alternative work considerations

The University has been working diligently to create a process to provide priority consideration for alternative work arrangements for employees who may be at a higher risk for severe illness from COVID-19 as defined by the Centers for Disease Control and Prevention (CDC). You can learn more about the specific eligibility definitions on the CDC website or by accessing the Step 1 request form below. The process is also available for employees who live in the same household with a person who is at higher risk under the same definition.
This process is separate from that for employees with disabilities requesting a reasonable accommodation covered by the Americans with Disabilities Act (ADA). Employees with disabilities who wish to request additional accommodations as a result of circumstances created by the COVID-19 pandemic should continue to use the ADA Request for Reasonable Accommodation or contact the Office of Accessibility Services at firstname.lastname@example.org or 419-372-8495.
If you or a member of your household are in the higher-risk population and you wish to request an alternative work arrangement, the process involves two steps:
Step 1: You will be asked to identify and describe alternative work arrangements that you feel may be needed to be able to perform the essential functions of your job. You will not be asked to identify a health condition or provide any medical documentation until Step 2. If the only risk factor you would be identifying is based on age, you will indicate that in Step 1 and will not need to complete Step 2. Step 1 must be completed by June 24, 2020, to receive full consideration.
Step 2: You will receive a follow-up email with a link to indicate the particular risk factor(s) for you and/or your household member and attach medical documentation. To ensure full consideration, you will be asked to submit the documentation required for Step 2 within seven days of receiving the Step 2 email. Any personal health information will remain secure and confidential and will not be shared beyond Accessibility Services or staff in the Office of Human Resources, as necessary. Please do not share personal health information or medical documentation outside of the Step 2 portal or with your supervisor. The Offices of Accessibility Services, Human Resources and the Provost or your supervisor will be in direct contact with you about the alternative work arrangements available in the weeks following your completion of Step 2.
After this initial process for priority consideration for alternative work arrangements for employees is completed by the Offices of Accessibility Services, Human Resources and the Provost, employees who are not in this higher risk group may seek alternative work arrangements with their supervisor.
