Supplies and Move Request Form
After you have conducted a workplace assessment of operations and physical spaces with the help of your supervisor or a designee, such as the Campus Operations building representative, and determined what safety supplies and signage are needed, you may submit your requests using this order form.
Please submit this form at least 10 days prior to the planned return to operations. You are encouraged to send the form as soon as possible after completing your workspace assessment to allow Campus Operations sufficient time to fulfill all requests.
Once your supplies and move request form has been submitted, a team member will follow-up with you prior to Campus Operations delivering the requested supplies, along with moving furnishings if needed and assisting in placing signage, etc.
