This is (pledge, active, alumni) state your full name of the Ohio Zeta chapter of the Phi Kappa Psi fraternity, founded February 19, 1852 at Washington-Jefferson College in Cannonsburg, Pennsylvania by Mr. William Henry Letterman and Mr. Charles Page Thomas Moore.
