Full-Time Student Verification Forms

Many insurance companies will require Full-Time Student Verification Forms to be completed for dependent children of enrolled members.  If you receive a request to update your full-time student status with your current insurance carrier, please fax these forms including your Student Id number to the Registration and Records Department at BGSU, (419)372-7977, located in room 110 of the Administration Building.
