372-9355. The dietitians are also available for lectures and group presentations.
What to Do Before Your Appointment: It is important to come prepared so that you can get the most out of your session. Please bring the following with you to
your first appointment:
- 3-day Food Diary - it is important to record what you eat and how much. Click here for a food diary
- Activity Log - we need to know how you exercise and how active you are during the day
- Goals - if we know exactly what you hope to achieve, we can make sure that we talk about how you can accomplish your goals
- Medical History - the more information you provide about your medical history, the better
