Eligibility

All currently registered students taking one or more credit hours are eligible for service. However, University employees taking courses paid for entirely by fee waivers are not eligible for care. Students can demonstrate eligibility by presenting student identification.

The spouse of an eligible student is also eligible for care. The following will be needed at check-in time upon the first visit:

  • Marriage certificate
  • The student's BGSU ID card
  • Spouse's drivers license or picture ID (e.g. passport)
  • Health insurance card
NOTE: Spouses who are covered by the BGSU Student Insurance plan do not need to pay this fee. Charges for a spouse's visit can be applied to the student's account.

For eligibility to use the SHS, the period between semesters is considered part of both the previous and the following semesters.

      Helpful Links:

A student registered for any summer term is eligible for the entire summer.

Students and spouses are eligible to use the SHS and the BGSU Pharmacy for one additional semester after the last completed semester.  Students and spouses will be responsible for charges not paid by their insurance company.

Any remaining refills on "previously filled prescriptions" can be refilled at the BGSU Pharmacy until the refills expire.

The following forms are in PDF format and readable by Acrobat Reader.  Please complete and return to the Student Health Service.

Health Assessment Form (Including Immunization Record and Medical/Prescription Insurance Information Form)

Students should bring their student identification card, their health insurance card, and pharmacy or prescription card each time they use the SHS.

Children of students are NOT eligible to use the Student Health Service for health care. Prescriptions for children of students can be filled at the BGSU Pharmacy.
