WELCOME to Bowling Green State University, Student Health Service (SHS), located in the Health Center building (#103 on your campus map).
Q. What immunization are required?
Required Immunizations:
- Measles (Rubeola) - Must have two measles immunizations or MMR injections, both after first birthday and at least 30 days
apart.
- Mumps immunization (must have two immunizations)
- Rubella immunization
- Tetanus, Diphtheria, Pertussis
- Completion of meningitis vaccination information
- Completion of Hepatitis B vaccination information
- TB Screening
Recommended Immunizations:
- Polio
- Hepatitis A
- Chickenpox (varicella) vaccine
Immunizations can be billed to the student's insurance or can be paid by cash, check, or credit card. The SHS accepts all
health insurance plans; if your health insurance plan covers immunizations, the SHS will bill the health insurance directly
for these charges.
Q. What information will the SHS require prior to treatment/immunizations?
- Student's BGSU Identification Card
- The following forms must be completed and returned to the Student Health Service:
- These forms must be on record (students under 18 years of age must have the Health Assessment Form signed by a guardian).
Note: these forms must be printed and then completed, signed and brought with you at the time of the first visit for treatment/immunizations, or
sent prior to the first visit.
- Health insurance card, pharmacy card and photo ID are required at every appointment.
If immunizations are not needed, please return these forms to the SHS by:
- bringing them to the SHS display table in the Bowen-Thompson Student Union during Orientation and Registration
- bringing them to the front desk of the SHS
- mailing the forms to: BGSU Student Health Service, Ridge Street, Bowling Green, OH 43403-0147.
Q. Are any immunizations or a physical examination required for entrance to BGSU?
BGSU strongly encourages, but does not require any immunizations or physical exam. We do encourage all students to keep their
immunizations up to date. Some international students may be required to have a tuberculosis screening test. Please click here for additional information.
Q. Is a student who is at BGSU for orientation & registration eligible to use the SHS?
Yes, students who are on campus during the summer for Oreg are eligible for all services from the SHS and the BGSU Pharmacy .
Q. What services does the SHS charge for?
Like most health care providers, the BGSU SHS charges for office visits and for the other medical services we provide - lab
tests, prescription medications, immunizations, minor surgery and medical supplies and equipment. We accept all health insurance
plans and will bill your health insurance company directly for all of the services that your health insurance plan covers.
The office visit co-pay will be applied to your student account only after the Student Health Service has received claim confirmation
from your insurance company. The general fee that you pay with your tuition every semester provides you full access to SHS
services and covers coinsurance and deductibles for office visits.

Q. Can I receive services at the SHS if I don't have health insurance?
Yes. The SHS provides services to all BGSU students regardless of their health insurance status or ability to pay. The SHS
strongly encourages all students to have and maintain health insurance coverage.
Q. Can I receive services at the SHS if I don't have the University sponsored insurance?
Yes, all BGSU students taking one or more credit hours are eligible to use the SHS.
Q. Does the SHS belong to my insurance plan?
The SHS is in network with over 80% of student insurance plans and will bill health insurance companies directly for medical
services provided in the SHS. Never let the lack of health insurance or an inability to pay for services prevent you from
getting needed health care at the SHS.
Q. Does the BGSU Pharmacy belong to my prescription insurance plan?
The BGSU Pharmacy accepts many third party prescription cards. We strongly encourage you to bring your insurance card to our
pharmacy while at Orientation so the pharmacist can make a copy for their files.
Q. Will the SHS file my insurance claim for me?
Yes. The SHS accepts all health insurance plans and will bill charges for the medical services you receive directly to the
health insurance company. Please bring your insurance card with you every time you visit the SHS.
Q. Will my insurance pay for charges at the SHS?
Most BGSU students have health insurance that will cover the costs of medical services (including office visits and ancillary
services, such as lab tests and prescriptions) provided in the SHS. The SHS accepts all health insurance plans and will bill
your health insurance company for the services you receive. The office visit co-pay will be applied to your student account
only after the SHS has received claim confirmation from your insurance company. If your insurance company will not pay some
or all of the cost of ancillary services (lab tests, prescriptions) you are responsible for the unpaid charges.
Q. What is a Co-Pay?
A co-pay is a fee that you are responsible for which is determined by the your insurance company. At the SHS, the office
visit co-pay will be applied to your student account only after the SHS has received claim confirmation from your insurance
company. Student's are responsible for co-pays on prescriptions and this must be paid (by cash, check, credit card or BG One
Card) at the time the prescription is received. You should check with your insurance company to find out more about your
co-pay amount.
Q. Does BGSU offer an insurance plan?
Yes, an independent student insurance plan is available through BGSU. For more information, visit the Student Insurance Office.
