Complete the student portion of theIntent to Enroll Form.
Identify a leadership mentor who has agreed to work with you or receive an assigned mentor from a list of those faculty and
staff members who have agreed to serve in this capacity (available through the Office of Campus Activities). You and your
mentor must both sign the BGSU LEAD Intent to Enroll form and submit this document to the Office of Campus Activities, 401

be used to determine your level of achievement within the program. Results of the leadership assessment will only be viewed
by the Leadership Advisory Board and Certification Committee unless you choose to request your results for personal reference.
Obtain a free copy of the LeaderSelf Resource Guide, available through the Office of Campus Activities. This will be a useful
resource as your progress through the BGSU LEAD Program, and may help to stimulate meaningful conversations between you and
your mentor.
Attend the BGSU LEAD Welcome Reception, held during the second month of each semester (see BGSU LEAD website for details).
