If you fall under Chapter 35, read below to learn what paperwork you need in order to file your claim. If you are eligible for Chapter 35 and have never received benefits from the GI Bill, you will need:

• BGSU Intent Form
•   VA Form 22-5490  

If you are eligible for this chapter and are a transferring student or have changed your major since the last semester in which you received benefits, you will need:

• BGSU Intent Form
• VA Form 22-5495
