If you will be receiving under Chapter 30 (active duty), Chapter 32 (VEAP), Chapter 34 (disability) or Chapter 1606 (Guard/Reserve), you will need to fill out the following forms:

• VA Form 22-1990 (application)
• Our Intent Form   

Please note that there are other things we will need before being able to process your certification.

If you are receiving under Chapter 30 or 34, we will need a certified copy of your DD-214.

If you are receiving under Chapter 1606 we will need a copy of your NOBE (Notice of Basic Eligibility) this should be available through your Guard or Reserve unit if you do not already have a copy.

If you will be receiving under Chapter 35 (dependent of an eligible veteran), you will need to fill out the following forms:

• VA Form 22-5490  
• Our Intent Form
