The application for readmission has two parts. Students re-applying to main campus need to submit both the readmission application and the Housing Office Data Information Card. Please submit the Housing Office
Data Information Card even if you don't intend to live on campus. Print both parts, complete, and mail or fax to the Office of Registration & Records, BGSU, 110 Administration Building, Bowling Green, OH 43403, fax number
419-372-7977. Remember, there is no application fee.
Students applying for readmission to the Firelands campus do not need to submit the Housing Office Data Information Card. Simply mail or fax the readmission application to the Firelands campus at the following address:
