Printable Application for Readmission
The application for readmission has two pages (front and back). All of the required fields may be completed on-line except the signature and date submitted lines. Please complete both front and back pages of the application, print it, sign and date, and then mail or fax to the Office of Registration & Records, BGSU, 110 Administration Building, Bowling Green, OH 43403, fax number 419-372-7977. Remember, there is no application fee.
Students who are applying for readmission to the Firelands campus should mail or fax the readmission application to the Firelands campus at the following address:
