New Employee Data Form

This form is used to load your information into the University’s computer system.  Please complete the entire form front and back.  Be careful when completing the Emergency Contact section.  The emergency contact should be a spouse, parent, sibling, child, grandparent, stepchild or significant other.  State of Birth should be the state where you were born.  Your Social Security Number is required on this document. Sign and date the back of the form. 

Employment Eligibility Verification (I-9)

This form is used to verify that you are eligible to work in the United States.  Please complete Section 1.  You will need to present two forms of identification to complete the verification.  The types that are allowed are printed on the back of the form.  Typically, a passport or a driver’s license and a social security card are used to verify eligibility.

OPERS Personal History Record

This form is used to enroll you in the Ohio Public Employees Retirement System.  Complete sections 1-4 only.  Once OPERS receives this form, they will mail you a designation form to select the type of retirement account you would like to have, as well as the beneficiary designation.  The OPERS New Member Handbook will outline the three types of retirement accounts that you can select.


Statement Concerning Your Employment in a Job Not Covered by Social Security

This document is used to inform you that Bowling Green State University is a state of Ohio public agency.  The state retirement system is OPERS and BGSU does not pay into social security.  You need to put your SSN on the top of the form, sign and date the bottom, and a copy will be given to you.

Form W-4 Tax Form

This form is used to determine the number of federal tax exemptions you would like to claim for payroll withholdings.  Please complete the bottom of the form beginning with the # (1).  You may keep the top section of the form to use for your personal records.

 

State of Ohio, Department of Taxation, Employee’s Withholding Exemption Certificate

This form is used to determine the number of state tax exemptions you would like to claim for payroll withholdings.   Complete the entire form and sign and date the bottom.  You may find your school district number by clicking HERE or leave this section blank if you need further assistance.

 

Green Deposit Information Form 

This form is used to establish your direct deposit for your payroll funds.  Please complete the form and attach a voided check or a preprinted deposit slip for your checking account. 

 

Drug Free Workplace, Sexual Harassment, Campus Security Act

BGSU has three policies of which you should be aware. The first is the Drug Free Workplace Policy, which states that if you are convicted of a drug violation while conducting university business either on or off campus, you must report it to the Office of Human Resources within five (5) business days.  The second is the Sexual Harassment Prevention Statement and Grievance Procedure.  All  employees are required to complete an online training program. The third is the Campus Security Act, which states that any member of the BGSU community may request crime statistic from the BGSU Department of Safety.   

 

DMA - Declaration Regarding Material Assistance/No Assistance to a Terrorist Organization

You are required to complete the DMA document when you sign your contract.  It is a pre-employment document.
