Welcome New Intermittent Staff!
This page provides access to the forms required to process your BGSU employment and payroll. This page also provides access to BGSU and governmental policies of which you should be aware.
Please select one of the following three options for completing these forms and printing policy and other documents. Please bring or submit the completed required forms and Records Checklist to the Office of Human Resources. Keep the policies for your reference.
1) Access and electronically complete and print the forms/policies from the table below. You will need to use
your home computer or a public computer to complete and print these documents. You will not be able to use
a BGSU office computer until your departmental budget data is loaded into our computer system.
2) Access and print a single PDF of the required forms/policies and manually complete using a pen.
Please use the Records Checklist as a guide to ensure that you complete all required forms.

3) Visit the Office of Human Resources and pick up a printed copy of the New Staff Member PDF Packet
and complete manually using a pen.
Regardless of the option you choose, we ask that you complete and submit these forms to your supervisor by the date he/she has specified.

| STEP | PURPOSE |
| 1. Complete the OHR Records Session Checklist | To ensure that you complete all required forms. Please check off the forms as you complete them and bring this checklist with you to your Records and Benefits session at OHR. |
| 2. Complete New Employee Data Form | To ensure that we have all data required by BGSU and the state and federal agencies. |
| 3. Complete Form I-9 | To verify that each new employee (citizen and noncitizen) is authorized to work in the United States. Please be certain to bring documents proving identity/employment authorization with you to your OHR Session. See page 5 of form. |
| 4. Complete Prior State Service Eligibility form | To determine if sick leave balances accrued from other State of Ohio agencies are eligible for transfer to BGSU. |
| 5. Print, complete, and sign the Policy Acknowledgement form/policy packet | To document that you received information on select BGSU and Federal policies and that you agree to abide by these policies. |
| 6. Complete (as needed) the Name Change Request form | To document a change in your name. Please note that you must sign this form in the presence of an OHR staff member and provide proof of identity with a Social Security Card or Passport. |
| 7. Print, complete, and sign the IT 4 form, Form W-4 and cover letter, and the Municipality Withholding form | To document the number of exemptions to be claimed for State Income Tax, for Federal Income Tax, and to request specific municipal withholding as needed. |
| 8. Print, complete, and sign the Direct Deposit Authorization form | To authorize BGSU to deposit your paycheck into any bank that is a member of the Federal Reserve. Please bring a voided check to verify the nine digit federal routing number. |
| 9. Complete the Ohio Public Employees Retirement System (OPERS) Personal History Record form | To document personal data, current BGSU employment information, and prior service information with the State of Ohio. |
| 10. Print, complete, and sign the SSA-1945 form | To document that you received information about the Windfall Elimination Provision and the Government Pension Offset Provision and the possible effects on your potential future Social Security benefits. |
