Health Care Benefits Information

Introduction

The benefits provided in the Health Care Program make up a significant portion of your overall compensation and could be considered your behind-the-scenes "second paycheck." BGSU believes it is very important for you to have a detailed resource explaining these benefits. The following sections provide information about your Medical, Dental and Prescription Drug benefits. Additional information can be obtained from the Plan Document which is available at the Benefits Office. In the event of a conflict between this document and the Plan Document language, the Plan Document language controls. The Board of Trustees of Bowling Green State University reserves the right at any time to modify or amend, in whole or in part, any or all of the provisions of the Health Care Program.

How to Use This Information

This following sections are to be used as a reference for BGSU employees to help them understand the Medical, Dental, and Prescription Drug Plans sponsored by the University. The first section, entitled "Understanding the Health Care Program," explains features such as eligibility and enrollment for you and your dependents. Utilization review, a health care management program, is also explained in this section.

Section II, "Your Medical Benefits at a Glance," is designed as quick reference to the Benefits provided under each of the optional Medical Plans.

Section III_A provides information about the covered and non-covered medical services for  Medical Plan A.

Section III_B provides information about the covered and non-covered medical services for Medical Plan B.

Section IV and V explain the major provisions of the Prescription Drug and Dental Care Benefits respectively.

Section VI, "Important Health Care Information," explains some of the important features of the University Health Care Program. How to file a claim; usual, customary and reasonable (UCR), coordination of benefits (COB) and continuation of coverage (COBRA regulations) are explained in this section.

This information concludes with Section VII, which contains useful definitions to assist in your understanding of the terms used in the Health Care Program.
