*Medical benefits are provided under a Managed Care Plan referred to as a Preferred Provider Plan (PPO). This plan provides benefits under a Preferred Provider Network of physicians and hospitals. A summary of this plan is available
at the The Office of Human Resources/Benefits department office or via our web page. BGSU has a working spousal rule which
is, if the employee's spouse works full time, the spouse's employer offers medical coverage and the spouse's employer pays
for at least 70% of the cost of health care, the spouse of the BGSU employee must pick up single coverage with his/her employer
for primary coverage and the BGSU plan will be secondary if the BGSU employee elects to cover his/her spouse.

* Plan pays based on usual, customary and reasonable rates at 100% for preventative and diagnostic services; 80 percent (after
$50 calendar year deductible) for basic maintenance and restorative, endodontic and periodontal services; 50 percent (after
deductible) for prosthodontic services. Maximum benefit of $1,500 per person per calendar year. Orthodontic benefits available
for dependent children up to age 19; lifetime maximum benefit payable of $1000.

Prescription Drug Plan

*Effective January 1, 2003, benefits provided under Caremark drug card plan which has network pharmacies. When using network pharmacy, the Plan pays 80 percent if the brand name is on the primary drug list and pays 60 percent if the brand name drug is not
on the primary drug list. (primary drug lists can change on a quarterly basis-see Caremark web site) The faculty member will
pay $4.00 for each prescription that is filled with a generic at the retail pharmacy and will pay $8.00 for the generic if
the prescription is filled through the mail order pharmacy. The prescription drug card program allows you to purchase up to
a 30-day supply at the retail pharmacy and up to 90-day supply can be purchased through mail order. Effective January 1, 2002,
there is a cap on the amount the faculty member will pay per script per fill at the retail or through the mail order pharmacy.
The cap is $100 per script per fill up at the retail pharmacy and $300 per script per fill through the mail order pharmacy.
Mail order pharmacy forms may be obtained in the Office of Human Resources.

Retirement

As a new member to STRS on or after July 1, 2001, there will be an option to choose one of three plans under the State Teachers
Retirement System. For members in STRS with 5 years or more service as of July 1, 2001, you remain under the Defined Benefit
Plan.
|
|
State Teachers Retirement System (STRS)
|
1. Defined Benefit Plan - Benefits received depend upon years of service, age at retirement from the system, highest three-year average salary and
payment options. The option also features survivor and disability protections, as well as access to health care coverage at
time of retirement.
Employee contribution to STRS - effective July 1, 2003, employee contributions for earnings on or after July 1 will be 10%
University Contribution to STRS -14% of earnings
2. Defined Contribution Plan - Under the Defined Contribution Plan, members make all investment allocations choosing from nine investment options managed
by STRS. Benefit amount depends entirely on contributions and investment gains or losses. The amount of benefits eligible
at retirement depend upon the total amount in the account. Members are vested immediately for their contribution and after
one year for the BGSU contributions.
Employee contribution to STRS - beginning July 1, 2003, employee contributions for earnings on or after July 1 will be 10%
University Contribution to STRS -14% of earnings (subject to legislative change)
3. Combined Plan - This plan option features components of both the defined benefit and defined contribution plans. Employer contributions
fund a defined benefit plan that offers retirement income at age 60 with at least 5 years of service, as well as survivor
and disability protection and access to health care coverage in retirement. Member contributions are deposited in a defined
contribution account to be managed by the member.

NOTE: University will pick up required faculty contributions and send this amount directly to STRS or the ARP carrier selected
on behalf of the faculty member, thereby reducing the taxable gross salary. The amount is deemed to be deferred income for
federal income tax purposes and is not taxed until ultimately received as part of STRS pension benefits or as an optional
refund at termination of employment in Ohio, or withdrawal from the ARP account.
University faculty and staff do not contribute to Social Security. However, individuals hired after April 1, 1986, do pay 1.45 percent of gross salary for Medicare
coverage.

Alternative Retirement Program

Note: An Alternative Retirement Plan (ARP) is a defined contribution plan which allows for immediate vesting of both the employer and faculty contributions. This option allows you to select one carrier
from 7 approved ARP carriers. Investment of money is controlled by the faculty member. The new full-time faculty member can
elect this defined contribution retirement plan in lieu of participating in STRS. Election must be made within 120 days from the effective starting date of contract.
Faculty Contribution to ARP - 10% of salary
University Contribution to ARP - 14% of salary and is subject to change. Please note that because of the 3.5% unfunded liability to STRS, only 10.5% of
the University's contribution goes to your ARP account.

Vacation is earned at the rate of 14 hours and 40 minutes per month (22 days annually) for those faculty members on a fiscal
year contract only. Faculty members with contracts for less than 12 months and those who receive time off with pay during
academic recess are not eligible for vacation.

Life Insurance and Accidental Death/Dismemberment

Employees may find it necessary to take leave from their jobs for a temporary period to address certain family responsibilities
or their own serious health conditions. BGSU complies with the Family and Medical Leave Act of 1993. This FMLA leave runs
concurrently with existing leave policies. Under the FMLA guidelines, the University will grant an eligible faculty member
up to 12 weeks leave during a 12-month period. Information packets are available in the Office of Human Resources.
