BGSU's open enrollment for Plan Year 2008 begins on Monday, October 22 and continues until 5:00pm on Friday,
November 16, 2007.

BGSU employees whose spouse has primary coverage under the BGSU plan must complete the Primary Coverage Spouse Form and return it by 5:00pm on Friday, November 16, 2007.  In the event the form is not returned, the spouse will be removed from coverage effective January 1, 2008 and will be offered single COBRA coverage that is a more expensive choice.

A Health Care and/or Dependent Care Reimbursement Form must be completed and returned by 5:00pm on Friday, November 16, 2007, for individuals wishing to participate in the health care and/or dependent care reimbursement accounts for calendar year 2008.
Health Care and/or Dependent Care Reimbursement Form

You must submit a BGSU Health Care Enrollment and Change Form if you are:

♦ making changes in coverage level such as adding or deleting a dependent
♦ changing marital status
♦ changing address (also needs to be changed through My BGSU)
♦ change in spouse's employment and/or health care coverage status
♦ waiving coverage if you have been covered
♦ electing coverage if you had waived coverage before
