This is where the specifics of your request are to be detailed. Any member of the support team can assist you or can direct
you to someone who can be of assistance in completing the form.
Send the completed form to the HCM or FMS team member listed at the top of the form.
The appropriate support / planning team will review the request and plan an appropriate course of action.
If the request involves a customization or enhancement, you will be asked to explain the issue to a group member so they can
write an Action Item for presentation to the Executive Steering Committee. The definition of a customization is available for your reference. Examples of approved Action Items are also available for reference.
Other requests, such as a reconfiguration of the system or data downloads, may involve attendance at a support / planning
meeting to provide further information.
Once the next steps are outlined, the issue will be added to the appropriate Issues List, assigned a priority status and target
completion date.
The support / planning team will involve the appropriate individuals as work is completed - input and feedback are essential.
