BGSU Outstanding Co-op or Intern of the Year Nomination Form

All students who are currently enrolled at BGSU and are participating or have participated in a Co-op or Internship during Summer 2012, Fall 2012, or Spring 2013 are eligible for nomination. Each organization may submit multiple student nominations; however, only one nomination per student will be accepted from any given organization. (Multiple nominators for the same student within the same organization should be submitted on a single nomination form.)

Scholarships are awarded based on the following criteria (readers will award up to 5 points for each criterion, 15 points total):

  • Work that goes beyond the employee’s job responsibilities and consistently exceeds established performance standards.
  • Informal and formal leadership by exhibiting work behaviors that support BGSU’s core values (Respect for one another, Collaboration, Intellectual and personal growth, Creativity and innovation, and Pursuit of excellence).
  • Exceptional service to the organization or community.
