Legal Service Fee Cancellation Form - If you choose not to participate, fill out this form and return it to the Bursar's Office no later than the end of the
second week of classes for each registered term.
All forms can be filled out directly online, printed off and submitted to:

Make sure to keep a copy for your records. Forms requiring a signature must be signed before they are submitted.
