To best submit your issue or request to the TSC, please choose from the topics listed below and then complete the form provided. Based on the information you provide in the form, a case ticket will be generated to address your problem, and you will receive an e-mail confirming the case ticket.

TSC Forms
University-Owned Computer Problem to report problems with a University-owned personal computer system.
Administrative System Problem to report problems with University-maintained mainframes and mainframe applications.
Administrative or Class Account Request to request accounts on BGSU Mainframes and servers.
On-Line Directory Information Update to update directory information for Faculty and Staff of BGSU only.
Podcast Account Request Form to request an account on the University's Podcast Server.
TSC Comment/Suggestion Form to give the TSC and Comments or Suggestions that you might have.
University Web Account Access to request accounts on one of the University Web servers.

Network Service Forms
New Networking Activation Request to request the addition of a new network jack or the activation of an existing jack.

Telephone Assistance Forms
Report Telephone Problems to report any problems with telephones and telephone lines.
New Service Activation to request the installation and activation of new telephone service.
Temporary Service Activation to request the installation and activation of temporary telephone service. Temporary service is limited to a maximum of 30 days.
Remove/Deactivate Service to request to discontinue existing telephone service.
Move Service to request to move existing telephone service.
Change Service to request to change existing telephone service.
Hosted Conference Call Request to request a hosted conference call be set up for your office or department.
Activate/Deactivate Department Long Distance Code to request or cancel a long distance code for a staff member in your office or department.
Change Display and/or Voicemail Assistance (Faculty and Staff only) to get assistance or report problems with voicemail accounts, change voicemail and/or request to change the caller id display on the telephone.
Voicemail Assistance (Students only) to get assistance or report problems with voicemail accounts.
For multiple move,add, or remove requests you can download and fill out an Excel spreadsheet and e-mail it to the TSC.
