Change Telephone Display and/or Voicemail Request Form
(Faculty and Staff only)
To submit a request to change the display and the voice mail on an existing telephone, please provide the information requested
below. This form must be filled out by an authorized representative of your department. Check with the department secretary to see
who is authorized. Based on information provided in this form, a case ticket will be generated. You will receive an email confirming the case
ticket. Your request will be completed as soon as possible.
