This web site contains PDF and HTML forms for business operations and services available to all BGSU Faculty and Staff. The offices contributing forms have approved each form. If you have similar forms that you would like linked from this website, please notify clining@bgsu.edu.

Acrobat PDF Forms

Most of the forms listed are Acrobat PDF forms. To use these forms, Adobe Acrobat 5 or Acrobat Reader 5 or a later version of those applications is required. The current version of Acrobat Reader can be download from Acrobat's web site. The PDF forms on this website are designed to be distributed electronically, filled in using a computer keyboard and printed. Fields that require a signature must be filled in manually before the form is submitted to the appropriate office for processing. These are some tips for using PDF forms:

  • The PDF form should display when the link is selected. If not, click the Refresh or Reload button on your browser window.
  • To enter information, the "hand" icon must be selected on the toolbar. If the "hand" icon is not depressed, click on it to select it. If the "hand" icon is not selected, the text I-beam will not appear in a text box so data can be entered. If the "hand" icon is selected and the form does not appear to be accepting any data, it is probably not a form but just a PDF file that you can print and enter information manually.
  • Click on an item to select it for data entry or use the tab key to move from field to field.
  • In order for items to appear when printing the form, each box must be deselected after the data is entered. As the form is filled in, just moving to the next data field will deselect the previous one. Click outside the last data field to deselect it.
  • The large boxes are for entering multiple lines of text. Data entered in those boxes will automatically scroll to the next line when the current line is full.
  • Some fields in a PDF form can have a predetermined format. When a dollar amount is requested, enter only the numeric value with the decimal point. When a date is requested, enter the month, day and year. If no year is entered, the current year will be supplied.
  • Values in some PDF forms are calculated. Data cannot be entered into calculated fields.
  • Some of our forms include internal and external links that can be identified by text that is surrounded by an outline. Click on the link to display the contents specified.
  • Some PDF forms include Print and Clear buttons. Clicking on the Print button brings up your print dialogue box so you can print the form. If you only want to print selected pages, enter those page numbers into your print dialogue box. The Clear button removes all data entered into the form. Neither the Print or Clear buttons will print when the form is printed.
  • Note : If you want to save the data you enter into the form, you must be using Acrobat (the commercial suite)! When you are using Acrobat Reader (the freeware) to fill in a form, all the data will disappear when you close the file. If you are filling out a long form, can't save it and can't complete it during one session, you can fill in one group of pages during one session and another group during another session.

HTML Forms

The forms in our title index specified as "HTML Form" are simple forms that send an e-mail message with the data entered into the form when the submit button is pressed.
