Applying to the Ph.D. Program

Those interested in enrolling in the Ph.D. program need to complete the necessary application forms below. Electronic submission of these applications is preferred, however, if you are unable to do so, you may complete and print the forms on these pages and return them to the indicated address below or fax them directly to Graduate Program Coordinator at 419-372-0366.

  • Three (3) Letters of Recommendation (click here to submit on-line recommendation letters)
  • Official transcripts
Copies can be faxed for evaluation purposes.  However, once student is admitted to the program, original transcripts must be submitted.
  • Official Graduate Record Exam (GRE) scores
  • Test of English as a Foreign Language (TOEFL) scores - international students only. If you have a copy of your TOEFL scores, please fax them to us at 419-372-0366. We do not need your official scores because we are able to verify them.
  • $30 application fee

    Fee can be deferred, however, will need to be paid after admission.
