1. The instrument is to be given in the last week of classes.

2. It is to be administered without prejudicial prefatory remarks made to the students.

3. The faculty member is not to be present while the evaluations are being administered.

4. The evaluations are to be administered, collected and returned to the department office by someone other than the instructor.

5. Exceptions to the last week provision are to be approved by the Chair.
