Here are some of most frequently used forms for journalism majors and minors. You can download them in Adobe Acrobat format
and then print them on your computer. These forms are also available in the School of Communication Studies office, 302 West
Hall.

Internship Policy (revised Spring 2009): Procedures and requirements for receiving journalism internship credit (JOUR 4000). Your advisor may have other requirements
as well. For help in looking for an internship or job, see our Internship and Job Search Guide.
Internship Application (revised Spring 2009): Complete this form and return to the School of Communication Studies office to apply for internship credit. Note that the
liability waiver on page 2 requires your signature. This form also requires your advisor's signature and the signature of
your professional supervisor.
Declaring a Sequence: Use this form to declare a sequence and request major status after completing JOUR 200. Please see your advisor for additional
information.
Independent Study: Complete this form in duplicate and return to the School of Communication Studies office to enroll in an independent study
course. Requires signature of supervising instructor.
Minor Form: Complete this form if you are declaring journalism as a minor. Requires advisor's signature.
