Below you can find descriptions and instructions regarding all forms and applications necessary for enrollment into the AYA Summer Program

Application: Print & submit by mail or in person

Host Family Profile: If you wish to stay with a host family, submit this along with your application

Student Consent, Medical Authorization & Release Form: This form is required for all students participating in study abroad programs and should be printed, filled out, and sent in with your application form.

Summer Program Student Conduct Form: Please print out, read, and fill out this form to acknowledge acceptance of BGSU rules in effect for summer program participants.

Please enclose a check for $120, made out to Bowling Green State University. This application fee will be applied to your program fee upon acceptance. It is refundable before June 1st.

If you have special housing-related requests, note them on the back of your application form and contact us to discuss them.

Non-BGSU students: If you are planning to take a language course to fulfill a requirement at your home institution, you should inform the appropriate advisor or office on your campus of your intentions. We will be happy to provide your advisor with information on credit options and detailed descriptions of each course level.
