Description
Undergraduate application for admission to CDIS Use this form to apply to the major after completing the pre-major requirements (CDIS 1230, 2240, 2250). This form should be submitted during the semester you complete these classes.
2011-2012 CDIS Checklist Download the checklist if you would like to evaluate the progress of your undergraduate program in Communication Sciences and Disorders. Use the checklist from the year you started in the CDIS program. Please contact your academic advisor if you have any questions. Your checklist is also available from the College of Health and Human Services Advising page.

Graduate Application Forms
Description
Graduate Program Checklist Use this form to determine eligibility to enroll in the graduate program in Communication Sciences and Disorders. This completed form must be submitted with your application.
Personal Letter of Application This instruction sheet should be used to guide your personal letter of application. You should complete the letter on your own word processing program, and send it directly to the department with your other application materials. Note that this is the only required personal letter of application. You should not copy this into the graduate school application. Formatting is lost and the graduate school application letter is not used.
Application for Graduate Assistantship Application for graduate assistantship. This form can be filled out and printed through Adobe Acrobat.
Letters of recommendation Use this form for letters of recommendation included in your application to the graduate program. Please fill out the attached form and give it to each person you are asking to write you a letter of recommendation. Then have them send the form and the letter (written on their own letterhead stationery) directly to us.
