submitted during the semester you complete these classes.
Download the appropriate checklist if you would like to evaluate the progress of your undergraduate program in Communication
Disorders. Choose the checklist from the year you started in the CDIS program. Please contact your academic advisor if you have any questions. Your checklist is also available from the College of Health and Human Services Advising page.
Use this form to determine eligibility to enroll in the graduate program in Communication Disorders. This completed form must
be submitted with your application.
This instruction sheet should be used to guide your personal letter of application. You should complete the letter on your
own word processing program, and send it directly to the department with your other application materials. Note that this
is the only required personal letter of application. You should not copy this into the graduate school application. Formatting
is lost and the graduate school application letter is not used.
Use this form for letters of recommendation included in your application to the graduate program. Please fill out the attached
form and give it to each person you are asking to write you a letter of recommendation. Then have them send the form and
the letter (written on their own letterhead stationery) directly to us.
