Graduate Program Application Process |

- Applying to our graduate programs in the Department of Communication Disorders requires two separate application processes, one for the department, and one for the Graduate College. On this page, you can download all the forms needed for the departmental portion of the admissions process. If you cannot
download and print the below forms, please send an E-Mail or written request to:

- To apply to the Department of Communication Disorders:

- Three letters of recommendation (downloadable form)
- NOTE: The letters of recommendation should be sent from each recommender to us directly, typed on their own official letterhead
and accompanied by this form.
- Personal letter of application (downloadable instructions)
NOTE: This instruction sheet should be used to guide your personal letter of application. You should complete the letter on
your own word processing program, and send it directly to the department with your other application materials. Note that
this is the only required personal letter of application. You should not copy this into the graduate school application. Formatting
is lost and the graduate school application letter is not used.

- To apply to the Graduate College:
Go to the Graduate College Admissions Web Site for instructions on what materials to send to the Graduate College, and on how to submit the required electronic graduate
application. Please note: The graduate college will need two official copies of transcripts from ALL institutions attended.
- International Students: Please note that you must send your materials to the Center for International Programs. Click here for more information on the process for international student applicants.
International students may be requested to provide the Department with a recorded speech sample. The Graduate Coordinator
will notify the applicant of specific instructions.

NOTE: All Departmental and Graduate College application materials (including transcripts and GRE scores) are due no later than February
15.
