The Pro Musica Grant Application is now a PDF.
Students: Click here to download the form and complete it. When you have finished the application click "Submit Application." Choose to either send the application via an e-mail program such as Entourage, or send it via webmail. The application will be sent to Linda Szych. ATTENTION MAC USERS - You must use Adobe Reader to complete and send this application. You can download Adobe Reader by clicking here or copy and paste this link in to your web browser: http://get.adobe.com/reader/. There is a section at the bottom titled "Faculty Recommendation." Do not fill this section out. This is for your faculty sponsor to fill out.
Faculty: When you receive a Pro Musica Grant Application it will now come from Linda Szych. You must:
1. Approve/Deny the application and at very end of the form
2. Provide comments if you wish
3. Type in your name (as a digital signature)
4. Type in the date
5. Save the form
6. Click "Submit Recommendation." Choose to either send the application via an e-mail program such as Entourage, or send it via webmail. The application will be sent to Linda Szych.
