Graduate students from the University of Toledo and the Medical College of Ohio who seek to enroll under the cooperative registration
agreement must submit a completed cooperative enrollment application for the program. Not completing this form may result
in holds on grades, registration, and transcript records. The application fee, admission fee, transcripts, test scores, and
letters of recommendation are not required.
