Description of the Activity:
Attached are two different sets of questions that can be placed at
the end of a test. In my class, to retake a test, I require the students
to do some intervention activity. The questionnaire gives me information
on what additional instruction is needed, information on their study skills
and strategies, and helps me choose an intervention activity. Most of the
information obtained from the questionnaire could be obtained from the
test alone. The questionnaire holds the student accountable for preparing
for the test. Through feedback on the questionnaire, the activity also
reinforces positive study skills.

Management Tips:
- Use open-ended questions
- Allow time at the end testing period for the students to completely
answer the questionnaire
- Don't accept one or two word answers
- Give feedback on questionnaire
