Engaging Youth Report
BGSU students who wish to volunteer:
Begin by filling out the Volunteer Information Form. This serves as your application to participate in the service learning course that is required for all students who wish to volunteer at the Teen Center. A limited number of applications will be selected for participation in the course. You may submit this form electronically to icfp@bgsu.edu
1. Volunteer Information Form
Once you've been accepted into the course: Fill out the top portion of the Independent Study Form, including your signature. Stop at the section entitled "Endorsements". This signed form must be hand-delivered (no electronic submissions) to 16 Family & Consumer Sciences Building no later than Wednesday, August 27, 2008.

Before you may begin work at the Teen Center: You must complete a background check. Students who have lived in Ohio for the last 5 years, use the form linked below. Bring proof of identity (Ohio Driver's License, birth certificate, OR social security card) and this form with you to campus police (Commons) to receive your background check. Campus police hours are 9am - 11am and 5pm - 7pm for these background checks.
3. Background Check Form (in-state students)
****Students who have lived out of state in the last 5 years need to go to the Wood County Sheriff's Office. These students must make an appointment. The Wood County Sheriff’s Office number is 419-354-9137. Please contact Niki Messmore at nmessmo@bgsu.edu to discuss payment for this service.****
Before you may begin work at the Teen Center: You must also fill out a schedule grid, listing all possible times you are available to volunteer at the Center. More detailed instructions are found through this link to the form.
