Instructions to fill in Sport Management Matriculation and Petition Forms

• Computers on campus are recommended to fill out the forms;
• The forms will appear much clearer if Dell PC are used as opposed to Macintosh, but either will work;
• You may need adjust the space when you fill in the form.
